How to order a Disclosure Statement
- Please complete and submit the online form below. You will receive confirmation when your request has been submitted successfully.
- Please pay the applicable fee, via bank transfer, to the account noted.**
- Click here to access the BCCM Form 12: Requiring Information from a Body Corporate. This writable PDF form must be completed before the Disclosure Statement can be issued.
- After you have submitted the online request form and paid the applicable fee via bank transfer, please email the completed BCCM Form 12 and confirmation of payment to marni@tcmstrata.com.
